We have recently been awarded £450 from the Port of Milford Haven Community Fund towards our Advance and Future Care Planning Service.  This grant contributed towards the costs of producing a film promoting the service, which is provided by registered nurses, working for the local charity.   

The grant, and non-cash support from Dooper.TV, has enabled us to produce a professional film.  This film will be a valuable resource used to showcase the service and highlight its benefits.  It will be used at Public Presentations; at Organisational Inductions to staff and volunteers and used by other health and social care professionals for education and signposting.

The film outlines the importance of considering your future care if there ever comes a time that you are unable to make your own decisions.  The Paul Sartori Advance and Future Care Planning Service is offered by experienced nurses, who are able to support conversations; give advice on the different types of documentation and can assist in drafting the paperwork.  They can liaise with the wider multi-disciplinary teams, should this be required, to ensure safe sharing of any advance care plan.  An advance care plan can enable a person to identify their values and wishes; share the information and record who they want to be involved in any future decisions.  It may include specific instructions about treatment or care choices as well.  For example, people may want to be cared for at home, in a hospice or in a hospital; they may have religious or spiritual beliefs that they would like to be respected.  The service has been supporting the local need since July 2015 and is a pivotal service for the charity.

An advance and future care plan can provide invaluable information and instruction for a family or loved one along with the healthcare provider, to make decisions often at emotive and difficult times, including end of life.

There is no cost to an individual wishing to undertake an advance care plan with Paul Sartori.  Individuals have to be registered with a GP in Pembrokeshire and the team accept referrals from all sources, if consent has been obtained.  The team do not provide legal advice.  Information can be provided and consultations arranged.  The team also support training and education to a whole range of organisations.  If you want to know more, please telephone them on 01437 763223 or email: acp@paulsartori.org.  The video can be viewed on our You Tube Channel.
