Clinical Nurse Specialist
For many patients in the late stages of their illness, palliative care needs are straightforward with District Nurses leading care of patients at home and Hospital Teams in hospital.
Physiotherapy
Palliative Care Physiotherapists enable patients to maximize independence with dignity. Physiotherapy helps patients and families manage life limiting illness and the symptoms these can cause, adapting to the changing nature of their condition.
Equipment Loan
The aim of the equipment loan service is to provide a rapid, short term loan service to patients meeting the Paul Sartori Hospice at Home criteria, in circumstances where sourcing through statutory channels would result in an unacceptable delay.
Counselling & Bereavement Service
Paul Sartori also provide an anticipatory grief service for children*. This is available to anyone below 18 years of age. We have to two qualified counsellors able to work with children. The sessions are a maximum of an hour long and take place here at Paul Sartori House, Winch Lane.
Complementary Therapies
This is a free service for adults in Pembrokeshire living with life limiting conditions and their carers, it is also available for bereavement referral clients. Clients are seen in their home, or at the Paul Sartori House Therapy Suite.

Advance Care Panning
n recent years, more and more people have made some kind of Advance Care Plan – what used to be called a ‘Living Will’. This is a document intended to be used if a person becomes unable to make decisions for him/herself.
