We specialise in end-of-life care at home, helping people remain in familiar surroundings or return home safely after a terminal diagnosis. We work closely with hospital teams, GPs, and community services to ensure a smooth and supported transition, so you are not left navigating this alone.

Helping You Understand
Hands-On Clinical Care
Our experienced nurses provide hands-on clinical care including symptom management, personal care, and support during the most difficult moments. End-of-life care focuses on comfort, dignity, and quality of life, and our team is there not only for the patient, but for you as a family, helping you understand what to expect and feel more confident in your role.
We also offer respite care, giving you time to rest and recharge while your loved one continues to receive compassionate end-of-life care at home. Even short breaks can make a meaningful difference during an intense and emotional time.
To help make care at home possible, we provide essential equipment on loan, including hospital beds, rise and recline chairs, and mobility aids. Having the right equipment in place quickly can make a significant difference to comfort, safety, and peace of mind.
Support for Patients and their Families
End-of-life care is not only physical, it is emotional, too. Our bereavement counselling and anticipatory grief support are available to both patients and their families, offering a safe space to talk, reflect, and begin to process what lies ahead.
We also offer complementary therapy for patients and carers, helping to ease stress, reduce discomfort, and create moments of calm when they are needed most. Our team can support you with Future Care Plans, helping your loved one express their wishes and make informed decisions about their care. Planning ahead can bring clarity and reassurance at a time when so much feels uncertain.
Above all, we are here to make things easier. To help you bring your loved one home, to support you in caring for them, and to ensure they receive compassionate, dignified end-of-life care, without any financial burden. Our services are available to people over the age of 18 living with a life-limiting illness in Pembrokeshire, and all support from Paul Sartori Hospice at Home is provided completely free of charge.
