When you need nursing support at home for your family member, you can speak directly to our team.
At Paul Sartori Hospice at Home, our Home Care Management Team is here to listen, talk through your situation, and arrange the right support for your loved one. You can call us on 01437 763223 during office hours, and you can leave a message at any time. If your call is received out of hours, we will return it as soon as possible.


For your family member
Care in Your Own Home
Our home nursing care provides hands-on support for people in the later stages of a life-limiting illness, helping them remain at home or return home from hospital with the right care in place. This includes support that can offer essential equipment and vital respite care for families during a difficult time. As part of our hospice at home service, our nursing support can also provide short periods of respite care, helping carers take a break while their loved one is cared for safely and with dignity.
Our nurses, including Registered Nurses and Nursing Assistants, provide care in the patient’s own home. This includes:
- Personal care, such as washing, dressing, and repositioning
- Comfort care and support with managing symptoms
- Sitting with your loved one so they are not alone
- Guidance and reassurance for family members and carers
We also provide night care
Flexible Day and Night Care
We can also provide night care, depending on assessed need, typically between 10.00pm and 7.00am. Daytime support may be offered on a short-term basis, which can include respite care for carers who need time to rest and recharge.
Our hospice at home service is designed to support families through the most challenging moments. Whether you are feeling overwhelmed, managing increasing care needs, or arranging someone to come home from hospital, our team can often respond at short notice.


Working Alongside Existing Care
We work alongside GPs, community nurses, and other healthcare professionals. Our role is to support and strengthen the care already in place, not replace statutory services. Where appropriate, we will advise that social services or continuing care support is also arranged.
There are some things we do not provide. This includes childcare, domestic tasks such as housework or meal preparation, routine tasks that can be carried out by regular home carers, and pet care. Our focus is on providing skilled nursing support and end-of-life care.
Who Can Refer
Referrals can be made by anyone. This includes family members, friends, patients themselves, as well as healthcare professionals such as Clinical Nurse Specialists, Community Nurses, and GPs.
Our services are available to people over the age of 18 living in Pembrokeshire who are in the later stages of a life-limiting illness. All support provided by Paul Sartori Hospice at Home is completely free of charge.
