During National Nurses Week, we at Paul Sartori would like to celebrate, not only the nurses who work to provide free care in Pembrokeshire; but the many other services that are available to support our patients and their families.
We offer a stand by service for people who are not currently in need of practical nursing care from us, but who may need assistance in the future. Once you are registered with the service, it will be easier to set up any required services at short notice.
This service starts with a home visit by one of our Healthcare Professional. At the visit they will assess whether you are eligible for the service and if you meet the criteria they will explain about all the services available to you. They will also tell you about any relevant services that may be of interest to you.
Once the assessment has been carried out all of our services are free of charge. Anyone can refer for this service, self assessment or family as well as District Nurses and any Healthcare Professionals. There are many services to help carers, who may not consider themselves as carers.
Our night respite service which allows the family to rest and recuperate whilst caring for their loved ones. This service is offered between 10 pm and 7 am
Our day respite nurses provide a hands on care service to enable someone who is caring for a loved one at home to take some time or attend their own medical or well-being appointments
Our on-call is available 24 hours a day 365 days of the year. You can speak to a member of our Care Management Team anytime about specialist nursing advice and symptom control as well as physiotherapy, counselling and complementary therapy

Our equipment loan service provide a rapid short term loan service for patients meeting the Hospice criteria. They loan larger items such as hospital beds, rise and recline chairs, bath hoists etc. Check out the range of equipment and see the team here
Our counselling team are ready to assist our patients, their families and carers, with all their counselling needs. They sometimes take outside referrals. For more information click here
