Paul Sartori Hospice at Home, a Pembrokeshire-based charity, were recently successful in obtaining a grant of £10,000 from The James Tudor Foundation. The Foundation, who support a wide and diverse range of health charities, awarded the grant towards the costs in supporting the Paul Sartori Registered Nurses. The team deliver end-of-life care to patients in the county, which amounts to over £200,000 per year.
The Nursing Team are experienced in end-of-life care and play an important part in the delivery of the holistic support offered by the charity. Paul Sartori offer a range of services to people living in the final stages of any life limiting illness, including home nursing care, complementary therapies, bereavement and counselling support, physiotherapy, future care planning and training. The home nursing team deliver thousands of hours of day and night respite per year to end of life patients in the community.
The Registered Nurses are crucial, playing an important part in supporting the patient and the family, which also enhances the provision of other local limited services. With their flexible and rapid approach, they support the patient’s wish to be cared for in their own home. They offer a patient-centred approach to care – assessing patients, referring to other internal services, work closely with other healthcare providers and are available as advisors. Nurses also assist with symptom management – administering medication when needed (to increase the comfort and wellbeing of the patient) and manage the 24/7 on-call facility (providing families with a point of contact in an emergency at all times of the day). The fast-track personal care package is coordinated by the team where they can support patients with care and equipment to ensure patients can return home from hospital.

With a Registered Nurse on site at night, patients may not need to wait as long for out-of-hours support or assistance,” added Laura.
